You must have FrontPage server extensions installed on your server for this FrontPage form to work and you must publish your web, not FTP it.

Make sure you change the email address in the "Form Properties" to your email address! In order to do this, right click on the form, select "Form Properties" > "Options". Select the tab, "Email Results," then change my email address ( marsha@templatesintime.com ) to your email address. Change also the name of the form under the same tab. Publish your web and your form results will come right to you!

If you get this message when you click on "submit":

You MUST call your internet host to have the FrontPage extensions reset. This form will not work without FrontPage extensions.
